GCSE History factors in Medicine through Time: how to weigh science, technology, government, war, individuals, chance and attitudes — with dates — instead of listing them.
Factors are a verdict, not a shopping list: which one unlocked the change in this period?

What you need to know

Science means a new explanation or method: Pasteur’s germ theory from 1861, Koch’s postulates, Landsteiner’s 1901 blood groups. Without a true cause, treatments aim at humours or miasma.

Technology means devices and industrial capacity: printing press (c.1450), microscopes, X-rays (1895), steam sterilisers, deep-tank fermentation for penicillin in the 1940s.

Government means law and money: 1853 Vaccination Act, 1875 Public Health Act (compulsory sewers and medical officers), 1946 NHS Act, wartime production contracts.

War concentrates wounds, urgency and budgets: Paré’s ligatures, Nightingale in the Crimea (1854–56), 1914–18 transfusion and Thomas splints, 1939–45 penicillin scale-up and the Emergency Medical Service.

Individuals matter when they change the file: Vesalius 1543, Harvey 1628, Jenner 1796, Simpson 1847, Lister 1867, Fleming 1928, Florey and Chain, Bevan 1948. Always attach the team or institution around them.

Chance: cowpox folklore, Fleming’s contaminated plate, forgotten chicken-cholera cultures. Chance only becomes history if a prepared mind and a system exist to use it.

Attitudes: Church defence of Galen, Gillray’s 1802 anti-vaccine cartoon, surgeons’ frock coats, BMA resistance to the NHS, public faith in cleanliness after cholera. Attitudes can block or spread a method.

The exam wants interaction. Germ theory (science) + phenol and sprays (technology) + Lister (individual) + slow professional uptake (attitudes). Your conclusion must say which was main for the question’s dates.
Quotations worth analysing
Short evidence. Real method.
“Treatments only jump when the explanation of disease jumps.”
This is a criterion for weighing science against other factors. Use it, then test it against 1875 sewers (miasma still helped) so it does not become a slogan.
“War is a terrible accelerator, not a kind teacher.”
War funds penicillin and blood banks while killing millions. Weigh it as speed and scale, not as moral progress.
“The state can pipe water before it understands bacteria.”
A limit to the “science first” thesis. Government sometimes acts on the wrong theory and still cuts deaths. That is why weighing needs a specific period.

Choose a criterion or your judgement is circular
“Science was important because it was important” earns little. Pick a measuring stick: necessity (could the change have happened without it?), timing (did it turn a possibility into an event?), and reach (elite wards or the whole population?). Example: science was necessary for targeting microbes after 1861; government was necessary for the 1875 Act’s pipes and for the 1948 NHS queue; war was necessary for 1940s penicillin tons. In 1250–1500, religion and classical authority outrank science because there was no germ theory to apply. A 16-mark answer that changes the main factor by period will beat a memorised order of “war, government, science” dumped on every question. State the criterion in the introduction; reuse it in the conclusion.

Worked interactions, not isolated pillars
Write links. Jenner’s 1796 method (individual + chance) needed the 1853 Act (government) against Gillray-era fear (attitudes). Lister (individual) needed Pasteur (science) and later steam sterilisers (technology). Florey’s penicillin needed Fleming’s plate (chance), Oxford biochemistry (science), and US tanks (war + government + technology). The 1875 Public Health Act used miasma arguments plus Snow’s 1854 map plus, increasingly, Koch’s cholera bacillus. When paragraphs end with “this shows science was important” they have not weighed. End with “this shows science supplied the target, but without compulsion the poor would still have waited for pipes”. That sentence is the difference between a list and an argument.

Common question types
“Main reason medicine improved after 1800” — germ theory as hinge, then NHS/1875 as delivery, war as accelerator. “How important was war” — strong for surgery and penicillin, weak for Jenner and Harvey, mixed for Nightingale (miasma hygiene). “How important was the individual” — necessary but never sufficient; always add communication and power. “How much change in the Renaissance” — knowledge versus treatment. Practise switching the named factor to the first paragraph, giving it a fair case, then beating it or confirming it with a criterion. Do not recycle the same three paragraphs regardless of dates. 1348, 1628, 1867 and 1948 are different problems.

Paragraph 3: 1853 and 1875 Acts plus NHS 1948 as delivery systems science did not build. Conclusion: without science, the state sanitises by luck; with science, it can aim. Science leads after 1700; it does not govern alone.
Exam technique
Turn knowledge into marks
If the question names a factor, that factor gets a full paragraph of fair evidence before you cut it down. A smear in three lines is not evaluation.

Can two factors share first place?
You can argue they were jointly necessary (science + government) if you explain the division of labour. Still finish with a tilt: which unlocked the possibility, which delivered it.
Where does religion belong after 1700?
As attitudes: opposition to Jenner, debates on chloroform in childbirth, later bioethics. It is no longer the main explanation of disease in the specification’s modern period.
Is chance a cop-out?
Not if you show the prepared context (Fleming in a lab; Jenner listening to dairymaids) and the system that scaled the accident. Chance as a magic wand is a cop-out.
How many factors should I cover?
Usually the named one plus two others, well evidenced. Four thin pillars lose to three weighed ones.
